Abstract

Biological width affects on the bone loss around implant and as a result it affects on pocket depth and BOP. Pus on probing may occur when peri-implantitis is progressed. So, the author makes wing structure on the top of implant and installed it as the top of implant located on alveolar crest and after compared clinical result between cis implant and general type implants. And also check the cytokines that are related with inflammation. IL-1β, MMP-2, MMP-12 was checked. The author gets less pocket depth with BOP level and find correlation with the finding of cytokine level. Probing depth in cis implant shows 2.074 ± 2.016 mm, and in general type implant 3.276 ± 2.091 mm (P < 0.05). BOP of cis implant is occurred 4 among the 61 implant and for general implant, BOP was occurred in 48 implants among the 87 implants (55.17%). Bone level difference in cis implants was 0.908 ± 1.176 mm, in general type implant was 2.509 ± 1.896 mm (P < 0.05). IL-1β of cis implants was 3.23600 ± 1.660060, in general type implants 4.94553 ± 4.679710, in normal gingiva 5.2713 ± 3.316547, and in inflamed gingiva 9.06986 ± 6.575085. MMP-2 of cis implants was 2.43527 ± 1.629108, in general type implants 3.02379 ± 2.715090, in normal gingiva 5.89583 ± 4.522338, and in inflamed gingiva 4.93683 ± 4.509219. MMP-12 of cis implants was 4.81556 ± 4.915005, in general type implants 3.58310 ± 4.884115, in normal gingiva 9.14640 ± 2.829904, and in inflamed gingiva 5.08586 ± 2.403748. As a results cis implant shows better in probing depth, BOP, and less cytokine expression then other comparing groups and also shows less bone loss then general type implants for the conclusion, wing typed implant has 1.0 mm longer biological width and this may roll on the protection of invasion of infection from the gingival sulcus to underlying bone.
